The pilot certainly hooked the wing to the crew twisted (as one connects in solo facing the wing.)... and made a mistake.
The APPI 7 point control procedure may not identify this problem. 

General opinion is that one should not hook to tandem twisted. With the tiredness, the stress that tandem operating generates, it is important to minimize sources of errors. 

It's recommended either to leave pilot harness-spreaders-wing connected, or connect the glider to the spreaders a straight way (glider in the back)

The pilot stores his gear, harness and glider disconnected. He hooks up to the glider on site according to one of these 2 sequences: either Pilot→Spreaders→Pax→Wing or Pilot→Spreaders→Wing→Pax. 
Considering the terrain and wind conditions, it is likely that he puts his glider side way (in position for cobra), stretches his lines, and at the end hooks the wing to the crew twisted (as one connects in solo facing the wing.)... and makes a mistake.

Control procedure errors still kill pilots. 
APPI recommendations are to check vital points one fully ready, JUST BEFORE taking off (not more than few seconds).

-Solo glider: 3 points mini
1 legstraps, 2 carabiners, 3 brakes

-Tandem operation: 7 points mini
1 passenger legstraps, 2 passenger carabiners, 3 main carabiners, 4 pilot carabiners, 5 pilot legstraps, 6 trimmers, 7 controls.

Tandem
Bad check procedure in tandem. Questions about gear maintenance with a handle that doesn't seem to hold in place.
After a missed take off, the pilot gets ready again and doesnt remark the reserve handle is hanging. Neither do any of the 4 assistant that help him take off.

Performing a preflight check just before takeoff is of the utmost importance

Quite often the pilot checks the passengers correctly and forgets his own carabiners and leg straps.

It often happens that the pilot forgets to check the steering lines. When one of them is stuck in a low position, as is the case here, the situation is particularly delicate: the opposite side must be seriously pulled to counter the rotation, which makes the paraglider difficult to maneuver and exposes it to stall if the angle of attack increases.

From the start of the action, given the conditions, the instructor is poorly positioned. When the pilot loses control, the paraglider quickly carries him away from the instructor, who can only be a spectator of the event.

Several points to comment on here

1/ choosing an approach where the student moves towards obstacles at the end of the base is not ideal

2/ using “right and left” may have caused the student to have a moment of doubt; it would have been better to use “go back to the landing” or “come towards me”

3/ By forewarning the student before the maneuver to be done, “you will enter the landing area before the silos,” this would have limited the chances of him considering something else.

4/ At the moment when the student hesitated, the instructor did not know how to regain control, but of course the situation was very tense, the instructor had difficulty judging with the parallax and the student should not have been sent into the silo.

One of the techniques widely used by instructors in schools is to get students to take off with a lot of braking. This has the advantage of limiting the risk of collapse, and limits the technical need for pitch control. In short, it allows to fly students with little work on the training slope. Apart from the fact that it gives pilots bad habits, it can be said that the sail is not very maneuverable and at the mercy of small gusts.
